INTAKE FORMS:
You will receive a link via email to fill out your online intake forms. Please fill them out upon receipt.
IN-OFFICE APPOINTMENTS:
Location: 1489 W. Palmetto Park Rd. Suite 410 (4th floor), Boca Raton, FL 33486
Please arrive 10 – 15 minutes early to your first appointment. Thereafter, arrive just a few minutes early. If you are late this will cut into your time and appointments still need to end on time. Couple therapy sessions will start when you are both are present to start your appointment.
Check in using the kiosk in the waiting room. If more than one person is in your session, only sign into the kiosk when you are both present.
This office building is located behind building address 1499. Follow the U-Health signage. Navigate into the parking garage. Once parked, take the centrally located elevator to the 4th floor. Look for “KHO suites” which will be on your left as you exit the elevator. Bathrooms are to the right of the elevator. If you need to use the bathroom, do so before or after your appointment as there is no bathroom in the actual office area.

ONLINE APPOINTMENTS:
Appointments are run via a HIPAA compliant version of Zoom. You do not need to download the program or have an account to join your meeting however you may if you wish and it is free (go to https://www.zoom.com/ to create a free account). Your Zoom link will come via email to you 1 day prior to your appointment.
Please follow these guidelines for your telehealth appointment:
- It’s essential to create a private and quiet space with minimal distraction for your appointment
- Sit in a well lit area (with light in front of you or above but not behind you)
- If in your car you must be stopped. Absolutely no driving during your appointment!
- Couples must sit together where I can clearly see you both, preferably from the waist up
- Couples who will not be in the same location together for any of your visits must discuss this prior to scheduling
- Join your appointment at your scheduled time. You may be in a virtual “waiting room” for a few minutes prior to being let into the meeting.

APPOINTMENT CONFIRMATIONS:
For appointments at the office, you will get a text to your cell number 1 day prior to your appointment. For online appointments, your email 1 day prior with the Zoom meeting link will be your confirmation. If you want confirmations to your email or at a different time, please let me know.
